This document presents the information requirements for Consumer Entered Notes. These are a logical set of data items for use in the My Health Record system and are therefore independent of any particular platform, technology, exchange format or presentation format.
Clarification: PCEHR means the My Health Record, formerly the "Personally Controlled Electronic Health Record", within the meaning of the My Health Records Act 2012 (Cth), formerly called the Personally Controlled Electronic Health Records Act 2012 (Cth).
