The Pharmacist Shared Medicines List (PSML) is a clinical document type supported by the My Health Record system.
The PSML contains detailed information about medicines the consumer is known to be taking when the list was created.
This includes both prescription and non-prescription medicines, including ‘over-the-counter’ (OTC) medicines and complementary medicines such as vitamins and herbal products.
What are the benefits of the PSML?
- Improved information sharing between health professionals. Transitions between episodes of care e.g. between acute and community care or community care to acute, are periods prone to risk and error
- Supports meaningful clinical use by providing pathways for the upload of PSML to the My Health Record system
- Provides timely access to medicines lists shared by healthcare providers via My Health Record
- Provides another source of medicines information available in My Health Record
- Helps to reduce medication interactions which improves both medicine safety and patient safety
How can the PSML help during medicines reconciliation?
The PSML contains the medicines the consumer is known to be taking at the time the list was created. It contains more than prescriptions and dispense records, which are already in the My Health Record system. The PSML may be an important outcome from a medicines reconciliation as well as providing a useful reference source for future medicines reconciliation.
For example, while performing a medicines reconciliation process, a pharmacist may identify a drug interaction between a complementary medicine such as St John’s Wort and a prescribed medicine (such as an anti-depressant) and intervene to resolve this interaction.
Who is responsible for creating and uploading the document?
Healthcare providers who are involved in the creation of a curated medicines list can prepare and upload a PSML to a consumer’s My Health Record.
Where does the information come from when creating a PSML?
A number of sources of medicines information may be used by the healthcare provider to create a PSML.
PSMLs might be created based on:
- a patient friendly hospital discharge medicines list
- a hospital discharge medicines chart (Interim Medication Administration Chart (IMAC))
- a medicines history provided by the consumer
- a dose administration aid medicines list
- a pharmacist professional service e.g. a Home Medicines Review (HMR).
A link to the PSML will also be provided in the My Health Record Medicines Information View that is presented in the National Provider Portal and National Consumer Portal.
What software products support the upload of PSML documents?
There are software products that have introduced or are working towards PSML upload capability. Hospitals are also increasingly supporting PSML document upload. We encourage you to enquire with your hospitals to find out more.
What changes to clinical information systems will be required?
Software vendors and healthcare organisations that provide My Health Record viewing functionality in clinical information systems will need to make minor configuration changes to their software before they can view the PSML documents from the My Health Record system. What do I need to do to view PSMLs?
Clinical information systems that view the My Health Record will need to ensure that the new document types can be accessed by their users. This will typically involve ensuring the new documents are a selectable document type on the document list screen. We recommend these documents are included by default.
Do pharmacy software vendors receive financial assistance for the PSML development?
The Agency is not offering any financial assistance to vendors wishing to help their clients upload PSMLs to the My Health Record. The Agency has a strong track record in the provision of documentation, source code and technical advice to ease technical barriers for software developers.
