The importance of digital health tools in improving the sharing of information across the healthcare sector is central to our work. Digital health tools can reduce the time spent accessing health history by streamlining information into one secure location. This seamless integration improves continuity of care and can potentially save valuable time, especially in emergency situations.
As part of this measure and to implement the Government’s response to Recommendation 68(link is external) (refer page 253), the Department of Health and Aged Care engaged the Agency to support the residential aged care sector to ensure older Australians receive better connected aged and healthcare enabled by increased use of the My Health Record system.
We are working closely with aged care Clinical Information Systems (CIS) and Electronic Medication Management Systems (eMMS) software developers to become conformant to the My Health Record system and support the use of the new Aged Care Transfer Summary (ACTS) document.

Aged Care Transfer Summary v1.0
The Aged Care Transfer Summary (ACTS) is a digital solution using My Health Record that facilitates access to crucial health information relating to an aged care resident to support clinical hand-over when an individual is transferred from an aged care setting to acute hospital care.
The ACTS implements a number of new record types into the My Health Record system for the sharing of residential health information from Residential Aged Care Facilities (RACF) systems. Learn more
