The workflow related to the management of patients with chronic diseases followed by a telemonitoring service is a cross-enterprise workflow. The telemonitoring process, and workflow related to it, may include a range of sharing infrastructures.
This supplement to the IHE Patient Care Coordination Technical Framework describes the Cross-Enterprise TeleHomeMonitoring Workflow Definition (XTHM-WD) profile, which sets out technical specifications for the standardisation of workflow in the telemonitoring of patients with chronic diseases.
