The Patient Plan of Care (PPOC) is an individualised, mutually agreed upon plan that nurses use to coordinate inpatient care. The PPOC profile describes the contribution of Registered Nurses to the plan and provides a mechanism for the exchange of data related to creating and managing individualised patient care. The plan includes problem issues (nursing diagnoses), expected healthcare outcomes, implementable interventions, and evaluation of progress toward outcomes based on follow-up assessment.
This supplement to the IHE Patient Care Coordination Technical Framework adds the PPOC Profile to Volume 1 of the Technical Framework and the Patient Plan of Care Document Content Module and related modules to Volume 2.
