This is the current version.
This document defines the specific implementations of established Patient Care Coordination (PCC) standards. The aim is to achieve integration goals that promote the exchange of medical information to coordinate the optimal patient care among care providers in different care settings. It is expanded annually, after a period of public review, and errors are regularly identified and corrected. This is the second of 2 volumes.
