Contraception is a major preventive health service that is not fully integrated or consistently captured within many electronic medical record (EMR) systems.
This document describes the Family Planning (FP) Profile, which provides a way to capture information about pregnancy intention and contraceptive method for the purposes of mandated reporting, monitoring and evaluation, and quality improvement initiatives related to family planning service delivery. The FP Profile defines the content of family planning information to be exchanged between systems.
