Contraception is a major preventive health service that is not fully integrated or consistently captured within many electronic medical record (EMR) systems.

This document describes the Family Planning version 2 (FPv2) Profile. This profile provides a way to capture standardised information about pregnancy intention and contraception for the purposes of mandated reporting, monitoring and evaluation, and quality improvement initiatives related to family planning service delivery.
