This standard describes the Nutrition Care Process (NCP). The NCP involves nutrition assessment and reassessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring and evaluation. The NCP standardises the process for providing nutrition care while allowing nutrition care to be individualised for patients and clients. The NCP standard is designed to improve the consistency, quality and predictability of nutrition care outcomes.
