The Patient-centered care team domain analysis model (DAM) is a model that captures the roles and relationships of the unique group of individuals who provide care for a single patient. Although this group of individuals may not have a business relationship, nor any relationship other than their participation in the care of the same individual, for the purposes of this DAM they are considered a team. The DAM helps produce standards to improve care and outcomes by creating an interoperable, computable representation of a patient-centred care team.
