This document describes how data from consumers' personal health devices (PHDs) can be transmitted to health systems using Fast Healthcare Interoperability Resources (FHIR). PHDs are often used in the home or on the patient's person. They are a key element in any remote patient-monitoring program. Commonly, a Personal Health Gateway (PHG) translates the PHD data into the appropriate form and uploads it to the receiving systems for electronic medical records, clinical decision support, and medical data archiving for aggregate quality measurement and research purposes.
