A continuity of care record (CCR) is a core data set of the most relevant administrative, demographic and clinical information about a patient's health care. It provides a means for one healthcare practitioner, system or setting to aggregate data about a patient and forward it to another practitioner, system or setting to support the continuity of care.
This document describes the Continuity of Care Document (CCD) specification developed collaboratively between HL7 and ASTM. It provides an alternative implementation to that specified in ASTM ADJE2369 for institutions implementing the HL7 Clinical Document Architecture (CDA). It describes the constraints on the HL7 CDA Release 2 specification in accordance with the requirements of ASTM E2369-05 Standard Specification for Continuity of Care Record (CCR).
