Healthcare delivery is complex. Patients, especially those with a history of multiple complex health issues, provide a care coordination challenge across institutions. Healthcare providers need a robust mechanism to clarify clinical findings, keep track of how comorbidities relate to and affect one another, and monitor the impact of different interventions on the progress of a patient’s conditions.
The Health Concern Domain Analysis Model supports care coordination considering the perspectives (concerns) of multidisciplinary providers across care settings. It provides a way to relate events to the health concern and enable events attached to the health concern to be monitored.
