Pressure ulcers are one of the most serious safety concerns related to hospitalisations, yet they are largely preventable.

Accurate identification of the risk of, and prevention strategies for pressure ulcers involves care coordination, patient health outcomes, care transitions, and effective preventative services.

This document describes a domain analysis model (DAM) for pressure ulcer risk assessment and preventative strategies. The DAM supports internal uses such as internal integration, quality control and decision support, and data exchange between providers and between providers and other stakeholders.

The model includes preliminary work on pressure ulcer interventions.
