Many score systems are used in health care on an international, national, hospital-wide or individual practice level. Other score systems are for scientific purposes.

The representation of assessment scales, scores or indexes in information technology can be based on the Detailed Clinical Models approach, which requires a high level of standardisation when gathering data in the clinical context. Assessment scales require strict interpretation of what is being measured (the variable) and vocabulary used to describe the same clinical concept (the value set) to maintain validity and reliability.

This standard describes the pattern an assessment scale should follow to enable uniform interpretation of scores for an assessment. It enables any valid and reliable instrument such as scales, scores, indexes to be exactly represented in the HL7 v3 environment, allowing adequate communication of such data.
